Renewal of Expired Passport
 
Consulate General of Pakistan, Chicago DOES NOT issue Machine Readable Passport (MRP) or process National Identity card for Overseas Pakistanis (NICOP) applications.
 
We only accept payment in form of Cashier's Check or Money Order
Please make a note a of tracking number of your return envelop before sending us your application
  • Complete Form-A (New Passport / Passport Renewal)
  • Four (4) current passport size photographs (2x 2) Standard US passport photographs
Photo should not be more than 6-months old
One (1) photograph must be notarized if sending by mail
Light blue or White background accepted
  • Original computerized national ID card (CNIC) or computerized ID card for overseas Pakistanis (NICOP) plus photocopy. **In case of its absence, old ID card and a copy of application for NICOP/CNIC is required for restricted validity passport.**  
  • Renewal fee is $46.00 for Ordinary (14 business days) or Urgent for $120.00 (5 business days) payable in Cashier check/ US postal money order to the Consulate General of Pakistan. All fees are non-refundable.
  • Expired passport in original with copy of pages 1-4.
  • Proof of residence in US - Notarized copy of green card OR valid US visa
  • Notarized copy of proof of residence in Illinois, Wisconsin, Minnesota, Iowa, Missouri, or Indiana. (Driver’s License or State ID acceptable)
  • Applicant under 18 years of age Pakistan born should provide ORIGINAL registration for FORM-B of NIC. **In case of its absence, copy of application for NICOP/CNIC is required for restricted validity passport.**  
  • The use of Express Mail is recommended for all deliveries. Self-addressed prepaid envelope should be included if sending by mail. Use United States Postal Service Express <USPS> Mail ONLY for urgent items. We DO RECEIVE envelopes though FedEx, UPS, DHL, and Airborne Express, but we CANNOT SEND return envelopes through them. Kindly ensure that return envelopes are of USPS only. Kindly note that the Consulate shall not be responsible for loss of documents in the mail. Please note the tracking number of your envelope prior to sending it to the Consulate.
NEW PASSPORT FOR CHILD
  • For children- Children will not be endorsed on Parent's Passports and should apply for separate passports for which the following requirements should be met if born in the US:
  • S-1 Registration form (issued within one year of birth in US)  **In case of its absence, old ID card and a copy of application for NICOP/CNIC is required for restricted validity passport.**  
  • Form A
  • Notarized copy of father's passport or US passport (of Pakistani origin) plus copy of either parent's Driver's License or State ID.
  • Four (4) current passport size photographs (2 x 2) Photo should not be more then 6-months old. One (1) photograph must be notarized if sending by mail. (Light blue or White background accepted)  
  • Renewal fee is $46.00 for Ordinary (14 business days) or Urgent for $120.00 (5 business days) payable in Cashier check/ US postal money order to the Consulate General of Pakistan. All fees are non-refundable.  
  • **NOTE: Passports with one year validity will be issued in the absence of NICOP, applicant applies for the same at the time of application for passport.
